About Standardizing eReferral Forms in Ontario 

In an effort to reduce administrative burden and improve clinician and patient experience across the province, and to support the goals of the ‘Patients Before Paperwork’ (Pb4P) initiative, Ontario Health is collaborating with Amplify Care to develop standardized eReferral forms. We are currently collecting feedback on 11 standardized eReferral forms (more to follow). 

eReferral Form Guidelines

  • All 11 specialty forms have been developed using a standardized structure and established best practices, ensuring that foundational elements are consistent across all forms. 
  • This standardized structure was co-designed in collaboration with consultant specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and OHTs.

Forms standardization has started with these 11 Forms. Ontario Health and Amplify Care are also working to conduct further analysis on other forms and will reach out as this work progresses. 

Senders (Primary Care Clinicians or Specialists)

Please review the various specialist forms that are of interest to you and complete the feedback survey for each form reviewed. 

Receivers (Specialists or Primary Care with Focused Practice)

Please review your specialty form and provide your feedback using the feedback survey.  Note: You are welcome to review additional forms.
