Standardized eReferral Forms in Ontario
In an effort to reduce administrative burden and improve clinician and patient experience across the province, and to support the goals of the ‘Patients Before Paperwork’ (Pb4P) initiative, Ontario Health is collaborating with Amplify Care to develop standardized eReferral forms.
There is an opportunity below to provide feedback on newly drafted forms (Open Clinical Review) and also existing forms (Annual Review).
1. Newly Drafted Forms (Open Clinical Review)

We are currently accepting clinical feedback on draft standardized eReferral forms for 5 Pediatric Subspecialties below. Please click on the links below to provide feedback on each of the forms.Â
Pediatric Otolaryngology (ENT)

2. Existing Forms (Annual Review)
The purpose of annual review is to ensure that Standardized Referral Forms (SRFs) remain current, effective and meet clinician needs by gathering, reviewing, and actioning feedback from a variety of sources. Annual review applies to live SRFs currently in use across Ontario Health referral pathways, helping ensure they continue to align with clinical practice, user needs, and evolving program requirements
Annual reviews are conducted using a phased approach. To date annual reviews have been completed for the non-clinical sections/common standard template as well as the Universal, Cardiology, Respirology, Hematology, Orthopedic, and Cataract Surgery SRFs.

eReferral Forms Guidelines
- All specialty forms are developed using a standardized structure and established best practices, ensuring that foundational elements are consistent across all forms.
- This standardized structure was co-designed in collaboration with consultant specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and OHTs.
Phase 1A
The Open Clinical Review period has ended, and 11 specialty forms are now complete.

Phase 1B
The Open Clinical Review period has ended, and three specialty forms (Cataract, Orthopedic & Medical Imaging) are nearing completion. Note: The links to these speciality forms are included below.
Phase 2
The Open Clinical Review for Endocrinology, Plastic Surgery, Infectious Disease, Psychiatry and Mental Health & Addictions PCA forms has ended.
Note: The links to the specialty forms are included below.
Phase 3
The Open Clinical Review for Suspicion of Cancer (BEACON), General Internal Medicine, General Pediatrics, General Surgery, Geriatrics, Neurosurgery, Sickle Cell, and Vascular Surgery has ended.
