Standardized eReferral Forms in Ontario
In an effort to reduce administrative burden and improve clinician and patient experience across the province, and to support the goals of the ‘Patients Before Paperwork’ (Pb4P) initiative, Ontario Health is collaborating with Amplify Care to develop standardized eReferral forms.

The Phase 3 Open Clinical Review on standardized eReferral forms for BEACON, Sickle Cell Disease Centres, Neurosurgery, General Internal Medicine, Vascular Surgery, Geriatrics, General Pediatrics, and General Surgery is now closed.

* BEACON (Building Enhanced Assessment for Cancer in Ontarians with Notable Symptoms) designates participating Cohort 1 sites as Suspicion of Cancer (SoC) clinical receiving sites to address a known gap for patients with persistent, progressive, or unexplained symptoms that raise concern for an underlying malignancy but do not yet meet criteria for a disease-site-specific pathway or specialty referral. These sites function as a new intake and coordination capability embedded within existing clinical programs, rather than as standalone services.
Note: The final eReferral forms will be available in both English and French.

eReferral Forms Guidelines
- All specialty forms are developed using a standardized structure and established best practices, ensuring that foundational elements are consistent across all forms.
- This standardized structure was co-designed in collaboration with consultant specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and OHTs.
Phase 1A
The Open Clinical Review period has ended, and 11 specialty forms are now complete.

Phase 1B
The Open Clinical Review period has ended, and three specialty forms (Cataract, Orthopedic & Medical Imaging) are nearing completion. Note: The links to these speciality forms are included below.
Phase 2
The Open Clinical Review for Endocrinology, Plastic Surgery, Infectious Disease, Psychiatry and Mental Health & Addiction PCA forms has ended. The links to the draft specialty forms can be found below.
