This toolkit includes Evidence2Practice Ontario’s complimentary EMR-integrated tools for heart failure, anxiety disorders and depression, diabetes management, and COPD – all in one spot!
The decision support tools offer a streamlined approach to support clinicians with appropriate assessment, diagnoses, and management, providing easier access to the necessary information at the point of care.
As an added bonus, Amplify Care has also included our Substitute Decision Maker tool and our Smoking Cessation tool to this package!
General updates for all conditions – September 2025
- Improved ‘Todays Vitals’ section in each tool visit form to now only display vital if captured on same day as form used. Updated mechanics to capture BMI.
- All forms now support ENCODE diagnostic coding.
- Improved UX alignment for updating and adding a diagnosis across all forms.
- Expanded ability to refresh updated screeners and vitals across all forms. Users no longer need to close and reload forms when a connected screener has been updated.
- Fixed issue where lab req labels were not aligned in the print view. Better support for sites to identify patients due for visits and improved form logic to support EMR queries.
Patient resources and care plan 1.3 release notes – September 2025
- Fixed issue where not all medications appeared in the patient medication section.
- French language support. Added links to French versions of existing patient resources.
Heart failure version 3.1 release notes – September 2025
- Improved ability to easily scroll to the latest instance of the SDM and care plan forms by clicking on the label.
- Ability to track adherence to quadruple therapy via EMR search.
- Updated ability to capture and record LVEF results and date. LVEF now able to be captured as a range.
Diabetes 1.3 release notes – September 2025
- Fixed issue where diabetes form would invoke outdated analytics forms.
- Added ability for users to track patients above A1C target via EMR search.
- Improved screening pathway to incorporate clearer calls to action and flags for at risk patients. Risk stratification using Canrisk algorithm built-into the EMR
COPD 1.2 release notes – September 2025
- Added ability to easily graph and trend CAT and nMRC values.
- Ability to refresh smoking status when updated
Anxiety disorders and depression 1.2 release notes – September 2025
- Added ability to eConfirmed diagnosis status labels and checkboxes on screening and visit forms reference the problem list for a coded condition (Snomed, ICD and Encode)
Heart failure version 3.0 release notes – January 2025
*NEW* Re-designed and updated heart failure tool. Key highlights include:
- Consolidated and condensed the content to provide a simplified experience to clinicians
- Localization features have been added that enable sites to customize the visit SOAP stamp, referral, and imaging links
- Ability to easily identify quadruple therapy and medication status for unique patients
- A comprehensive Management Hub with panels for diagnostics, medication reconciliation, and billing
- An updated diagnosis section for precise ejection fraction-based classification
Additional updates:
- Support for adding ENCODE to diagnosis now available
- Dry/target weight now captured as a custom vital
Anxiety disorders and depression version 1.1 release notes
When a confirmed diagnosis is added to the screening tool when the tool is reopened or refreshed the confirmed diagnosis is shown as anxiety.
- Fix: Each diagnosis pulls in the appropriate/selected code to the problem list

Screening: Checkboxes for suspect conditions are unintentionally deselecting previously checked items.
- Fix: Each checkbox is acting independently of another. Users can insert a date associated with a suspect condition. If a diagnosis is made after a condition is deemed suspect, the form will retain the date and checkbox condition in the suspect column.

- Fixed issue with graphing of certain lab values
- Canrisk score is now captured as a custom vital @CANRISK. It is recommended to re-enter the most recent score so that moving forward these values can be viewed and graphed easily.
- Canrisk score now displays the latest value and date upon form open
- Added confirmation flag when clearing form values
- Added confirmation flag when updating visit frequency
- Addressed issue where diagnosis status checkboxes were not all mutually exclusive
- Form will now collapse when added to progress notes and chart is refreshed
