Improving the quality of patient care

Knowledge Translation and Evaluation (KTE), formerly known as Benefits Realization, is a cornerstone of all programs led by Amplify Care. Our goal is to ensure that digital health technologies enable best practices and drive improvements in patient care quality. This work is supported by a model published in Building Capacity for Health Informatics in the Future, a peer-reviewed eBook (ITCH 2017), and is closely tied to our change management and adoption processes.

A group concept mapping study

Use of an electronic wellness instrument in the integrated health and social care of older adults.

The aim of this study was to develop strategies to leverage an electronic wellness instrument, interRAI Check Up Self Report, to support integrated health and social care for older adults and their care partners in a community in Southern Ontario, Canada.
