We work with clinicians to develop condition-specific tools, ensuring they fit seamlessly into workflows and suit your needs. The E2P tools are also fully customizable and modular, providing an agile approach.
Integrated forms for easy access and EMR immunization searches
We make it easier for clinicians to find patients who are eligible or due for immunizations, as well as access forms directly from their EMRs.
How to implement
Step 1
Select the tool(s) that interest you from the list below (you can filter by EMR and other options)
Step 2
Fill out the form to download the tools.
Step 3
Upload the file into your EMR – for E2P tools, there is an implementation guide for support (these guides vary according to each EMR).
Step 4 (optional)
Set up a time with one of our Change Management Specialists who can walk you through optimal use of the tool.

In partnership with the Centre for Effective Practice and North York General Hospital, Amplify Care (formerly eHealth Centre of Excellence) is proud to be co-leading Evidence2Practice Ontario.
Evidence2Practice is working with clinicians, topic experts, and other system partners to develop a suite of free evidence-based tools that integrate seamlessly into frontline systems, enabling acute and primary care clinicians to access the information they need easily and quickly at the point of care. The tools also streamline patient journeys by including connections to community resources and patient self-management resources.
