Standardized eReferral Forms in Ontario
In an effort to reduce administrative burden and improve clinician and patient experience across the province, and to support the goals of the ‘Patients Before Paperwork’ (Pb4P) initiative, Ontario Health is collaborating with Amplify Care to develop standardized eReferral forms.
The Open Clinical Review is now closed. All feedback is currently being reviewed.

eReferral Forms Guidelines
- All specialty forms are developed using a standardized structure and established best practices, ensuring that foundational elements are consistent across all forms.
- This standardized structure was co-designed in collaboration with consultant specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and OHTs.
Phase 1A
The Open Clinical Review period has ended, and 11 specialty forms are now complete. (Note: please see links to the speciality forms below).
Phase 1B
The Open Clinical Review for standardized eReferral forms for Cataract, Orthopedics and Medical Imaging (CT, MRI, Ultrasound, X-Ray, Bone Mineral Density) is now closed.
Phase 2
Standardized eReferral forms are currently being built for additional specialty areas. The Open Clinical Review is expected to take place in September 2025.
